Heartfelt Connections–A Memory Care Program

Gable Pines features the renowned Heartfelt Connections–A Memory Care Program®, a person-centered approach grounded in the belief that the abilities that remain are far more important than those that are lost.

Our team employs reminiscence tools to help residents’ family members and loved ones capture the details of residents’ lives—their careers, hobbies, likes and dislikes. We then apply the insights gained from each life story to create an individualized care plan that promotes physical and emotional well-being, prolongs daily functioning, and helps maintain a sense of purpose, satisfaction, and quality of life.

Frequently Asked Questions

To some degree of memory loss is normal with aging. To be considered dementia, symptoms need to affect more than one area of brain function significantly enough to interfere with everyday life. Memory loss is a common example. As you’ve noticed, your loved one may have difficulties forming new memories. Communication is another key area. Your father may experience challenges in processing speech and language. Changes in mood are common: depression, apathy, or a change in personality. He may be confused, or be challenged in his sense of direction and spatial orientation. He may experience a decline in judgment – his ability to consider facts and come to a reasonable conclusion. You may be in a good position to observe and monitor subtle changes such as these in your loved one. It’s also a good idea to talk with your father’s doctor and share your concerns.

Progressive dementia is the most common type, with five progressive stages

  • In Stage One, there is no impairment. There is no significant memory problem, judgment is normal, and your loved one is fully able to care for their personal needs.
  • Stage Two is characterized by slight impairments, such as memory inconsistencies and struggles with timing or solving problems. Your mother can still manage personal care without any help.
  • Stage Three is a noticeable but mild impairment in areas such as short-term memory, disorientation and getting around. Chores may begin to be neglected. Reminders are needed for such things as personal hygiene.
  • A person with Stage Four dementia has moderate impairment. Though well enough to go out, your mother would need to be accompanied for social activities and chores.
  • Stage Five is characterized by severe impairment – the inability to function without help. Stage Five is characterized by extreme memory loss, confusion and lack of orientation. At this stage, everyday functions are almost impossible, even with assistance.

While both assisted living and memory care provide assistance with the activities of daily living—such as help with bathing, dressing, grooming, and medication management—memory care at Gable Pines offers a specific type of compassionate Alzheimer’s care and dementia care that is specially designed for those with memory impairment. In addition, memory care residents live in a specifically designated “neighborhood” designed for safety, security, and socialization.

Memory care at Gable Pines provides a specially designed, whole-person approach to Alzheimer’s care and dementia care. Residents live in a warm and friendly, family-like neighborhood with the safety of 24-hour onsite staffing and the loving care of highly trained personnel who implement proven memory care programs that encourage and celebrate remaining abilities. It would be virtually impossible to duplicate this level of round-the-clock safety, sense of community and socialization, and specific expertise in Alzheimer’s care by using an in-home care service.
